With life expectancies 15 years below other neighborhoods in Louisville, the Smoketown Family Wellness Center Pediatrics (SFWC) has a dream that every child will grow up healthy, living up to their potential regardless where they live. This year we've added several services based on our families needs, including our Impact 100 community health worker, a minivan to transport our families to and from medical appointments and a full time therapist to provide longterm, ongoing counseling services.

The SFWC model of healthcare delivery moves care upstream to provide primary prevention of diseases that begin in childhood while providing excellent pediatric care. We support parents with the tools they need to raise their children to be healthy in mind, body and spirit. The Center provides clinical care for children as well as essential needs and healthy lifestyles in a supportive community-based setting. Families are health experts themselves, spreading their knowledge to family, friends and neighbors, thus creating a positive culture of health and wellness.

Insurances and Medicaid pay for traditional medical services and we rely on grants and donations to allow us to provide wrap around services to improve overall wellbeing. We use Give for Good Day as our annual fundraiser in order to take advantage of the many financial prizes and incentives to maximize our fundraising.
